1. In consideration of the recording and preservation of my oral history by Women .
Trailblazers in the Law, a Project of the ABA Commission on Women in the Profession
(hereinafter the “Project”), I, Fern M. Smith, hereby assign, grant and convey to the
Project and its successors and assigns, all of my rights, title, and interest in and to such
oral history, including the recordings, transcripts, and computer diskettes and/or CDs of
my interviews, as described in Schedule A hereto, and copyrights and other literary and
intellectual property rights, except as reserved in paragraph 2 of this document.
2. I reserve for myself and to the executor of my estate a right to use the recordings,
transcripts, and diskettes and/or CDs and their content as a resource for any book, article
or other writing of which I or my executor may be the author or co-author.
3. I authorize the Project, its successors and their licensees to duplicate, edit,
publish, including publication on the Internet, or permit the use of said oral history,
recordings, transcripts, and diskettes and/or CDs in any manner that the Project considers
appropriate, and I waive any claims I may have or acquire to any royalties from such use.
4. I authorize the Project, its successors and their licensees to use my name, likeness
and biographical information to indicate my role in the creation of said oral history,
recordings, transcripts, and diskettes and/or CDs.

be in the form as set forth within this Jurat. There are no exceptions. If a Jurat
to be completed does not follow this form, the notary must correct the
verbiage by using a jurat stamp containing the correct wording or attaching a
separate jurat form such as this one which does contain proper wording. In
addition, the notary must require an oath or affirmation from the document
signer regarding the truthfulness of the contents of the document. The
document must be signed AFTER the oath or affirmation. If the document was
previously signed, it must be re-signed in front of the notary public during the
jurat process.
• State and County information must be the State and County where the
document signer(s) personally appeared before the notary public.
• Date of notarization must be the date that the signer(s) personally appeared
which must also be the same date the jurat process is completed.
• Print the name(s) of document signer(s) who personally appear at the time of
notarization.
